Waiver and Release of Liability - {name}

I, the undersigned, being the parent or legal guardian of the above-named minor, hereby give my permission for my child to participate in activities, training sessions, and programs (collectively referred to as "Activities") at EmpireMMA (the "Gym"). I acknowledge and agree to the following:

1. Assumption of Risk
   I understand that participating in training activities at the Gym, including but not limited to weightlifting, cardio exercises, martial arts, and other physical fitness programs, carries inherent risks of injury. These risks may include, but are not limited to, physical injury, illness, or even death. I voluntarily assume all risks associated with my child's participation in these Activities.

2. Medical Fitness
   I confirm that my child is physically fit, has no known medical conditions that would prevent them from safely participating in Gym Activities, and has been cleared by a medical professional if necessary. I agree to inform the Gym of any health conditions, allergies, or physical limitations my child may have before participation.

3. Waiver of Liability
   In consideration of my child’s participation, I hereby release, waive, and discharge EmpireMMA, its owners, employees, trainers, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may occur as a result of my child’s participation in Gym Activities, whether caused by negligence or otherwise.

4. Indemnification
   I agree to indemnify and hold harmless EmpireMMA, its owners, employees, trainers, and representatives from any claims, damages, liabilities, or expenses (including attorney's fees) that may arise from my child’s participation or any actions taken by my child while on Gym premises.

5. Emergency Medical Treatment
   In the event of an emergency, I authorise the Gym and its staff to seek medical treatment for my child. I understand that I am responsible for any medical expenses incurred as a result of injury or illness during participation.

6. Parental Acknowledgment
   I acknowledge that I have read and fully understand the terms of this waiver and release. I understand that by signing this document, I am waiving certain legal rights, including the right to sue the Gym or its staff. I agree that this waiver is binding on me, my child, and our respective heirs, legal representatives, and assigns.
